Please download the department's enrolment form enrolment form (PDF, 5.5MB) and return to the school. If you are unable to download the form, please
contact us and we will provide you with a copy. You are also required to submit the following forms:
An original, full Birth Certificate (for students not previously enrolled in an Education Queensland school)
Provide current passport and visa for any child born overseas
Provide copies of any relevant custodial documents and/or court orders.
